Adam-Hakim Syndrome – Normal Pressure Hydrocephalus (NPH)

Adam-Hakim Syndrome is a condition that typically affects older adults. It occurs due to the accumulation of cerebrospinal fluid (CSF) in the brain’s ventricles, even though the CSF pressure may be normal. This condition can cause symptoms that resemble other neurological disorders such as Alzheimer’s disease or Parkinson’s disease, making accurate diagnosis by a specialist essential.

The classic triad of symptoms:

  • Gait disturbance: Slow or difficult walking, with short, unsteady steps

  • Cognitive decline: Memory problems, reduced mental abilities, or forgetfulness

  • Urinary incontinence: Loss of bladder control

Diagnosis:

  • Clinical examination in the clinic to evaluate symptoms and their impact on daily activities

  • Brain MRI to assess ventricular enlargement

  • Lumbar puncture (CSF tap test) to measure improvement in symptoms after temporary CSF drainage

Treatment options:

  • The main treatment is placement of a ventriculoperitoneal (VP) shunt to drain excess CSF into the abdominal cavity, which helps to:

    • Significantly improve walking

    • Restore some cognitive abilities

    • Regain control over urination
